POLICIES & PROCEDURES
Training sessions must be scheduled no less than 7 days in advance. Days, dates & times need to be planned out during the consultation to allow trainers to plan for other clients. *If you are unable to schedule all sessions, you must be scheduled in one week time frames.

I acknowledge that I have been given the opportunity to ask questions and gain clarification where needed. Checking this box reflects my agreement to the policies and procedures and I understand that I will be held accountable for this information moving forward, while participating in the Personal Fitness Training Program.
